Andrew Poole, 56, was working at a farm when dodgy equipment saw him catapulted from his seat.
The
driver's seat was sheared off the main part of the machine causing him
to fall backwards while still connected to the chair, a court heard.
The roller continued to reverse over him - fatally crushing his head, in Colerne, Wiltshire.
An
investigation by the Health and Safety Executive revealed the roller
was in a poor condition, the safety cut-out switch was broken and
emergency stop button was missing.

His employer
Globareward Limited was fined £10,000 and ordered to pay costs of
£30,750, after pleading guilty to an offence under the Health and Safety
at Work Act.
Director Paul Thomas Andrews was
sentenced to two months in prison suspended for two years and ordered to
do 200 hours community service at Swindon Crown Court.
The court also heard Mr Poole, who worked for the company based in Chippenham, Wilts., was untrained.
